Doel en doelgroep:
This project aims to evaluate the effect and efficiency of the Systematic Tool to Reduce Inappropriate Prescribing Assistent (STRIPA) in medication reviews.

Relevantie voor de beroepspraktijk/maatschappelijke relevantie:
Polypharmacy is associated with negative health outcomes because of interactions between drugs, wrong drug use and adverse events. Each year there are approximately 40.000 drug-relate hospital admissions in the population of older people. Several interventions for safer medication care is needed. One of the intervention is the yearly medication review. Implementation of the medication review is not structurally embedded in practice due to several factors (e.g. insufficient of knowledge by performers of the medication review, insufficient collaboration between general practitioners and pharmacists and insufficient imbedding of the review in work processes in primary care). The medication review is experienced as time consuming whereby reviews on large scale is impossible. The STRIPA method reduces the time load and consultation moments for each patient between care providers and pharmacists.

Resultaat/producten:
Research questions are:
1. What is the effect of the farmacotherapie in terms of the amount and the clinical impact of farmaco therapeutic problems by using STRIPA in medication reviews compared to medication reviews without STRIPA?
2. How do patients experience optimalization of the farmacotherapy after implementation of STRIPA medication reviews compared to medication reviews without STRIPA?
3. How do general practitioners, pharmacist and nurses experience STRIPA in terms of barriers and facilitators?

Results are expected in November 2019.

Implementatie:
If the STRIPA method for medication reviews is shown to be effective and efficient, STRIPA method will be implemented on large scale.
